What do I need to bring to my appointment?

Insurance card, ID, all your medications in the ORIGINAL bottles.
If you are a new patient, make sure to arrive at least 15 minutes early to fill out the necessary paperwork.

How do I pay for my services?

If you have insurance, please bring in your card and we will either collect your copay or deductible amount and bill through insurance. If you are not insured, full payment is due at the time of service. We accept debit/credit cards, checks and cash.

What is your cancellation policy?

We require 24 hours' notice to cancel appointments. If you do not cancel your appointment and not arrive at your scheduled appointment time, you will be charge a $25.00 NO SHOW fee.

How do I contact my provider?

Call the office at 806-481-7000 and leave a message or schedule an appointment.

How do I obtain my medical records?

A signed release is required and can be signed in our office and must be signed by the patient, parent or legal guardian.

How do you notify patient of lab and/or test results?

Patients are required to schedule a follow up appointment with one of our providers to review any lab reports, X-rays, MRI, CT and US results.

Do you offer same day or urgent appointments?

We strive to set aside time for urgent visits, however we cannot guarantee same day appointments. If you have a medical emergency call 911 or go to the nearest emergency room.

Weight Loss Program FAQs

Does insurance cover this?

No this program is self pay only but we do accept all major credit cards.

How do I know if this is right for me?

•You've tried everything and nothing seems to work
•You're constantly tired and unmotivated
•You'd like to lose weight and decrease cravings

•You want you and your family to learn healthy eating habits

What should I expect?

•We will explain how food affects your health and which foods are significant in your diet
•You will receive a personalized eating plan along with recipes and suggestions for your diet
•We will work with you to set goals for you to apply into your diet

I am on a variety of medications: can I still participate in the program?

We have found that very few medications negatively interact with the medical weight loss Program. When you come in for your consultation, the practitioner will thoroughly review your medical history and discuss it with you.

How often do I have to visit?

It differs. During the early stages of the weight loss program, you will visit on a weekly basis. Once you have reached your weight loss goal and transitioned into Maintenance , you will visit on a monthly basis.
